Case Study: Resolution of Severe Low Back Pain with Conservative Chiropractic Care and Nonsurgical Spinal Decompression

Chief Complaint:
R.M. presented with severe low back pain that significantly impaired mobility and activities of daily living.
History:
The patient had worked in a sedentary desk job for approximately 40 years. The day prior to the onset of symptoms, R.M. participated in a Topgolf outing. Shortly thereafter, severe low back pain developed, making routine movements difficult.
Initial Examination:
Upon presentation, the patient demonstrated significant functional limitations and objective clinical findings, including:
- Forward antalgic posture
- Difficulty walking
- Difficulty assuming and arising from a seated position
- Difficulty assuming and arising from a lying position
- Active lumbar range of motion severely limited by pain in all planes of movement
- Positive Kemp’s test, reproducing severe lumbosacral pain
- Marked lumbar paraspinal muscle spasms
Diagnostic Imaging:
Lumbar motion-study radiographs revealed:
- Lumbar disc degeneration
- Lumbar facet arthropathy
- Severe limitation of intersegmental lumbar mobility
These findings were consistent with mechanical dysfunction of the lumbar spine contributing to the patient’s symptoms.
Treatment Plan:
A conservative treatment plan was implemented consisting of:
- Chiropractic adjustments directed to the lumbar spine
- Nonsurgical spinal decompression therapy
- A customized home stretching and therapeutic exercise program
The patient was treated twice weekly for eight weeks and remained compliant with both in-office and home-care recommendations.

Summary:
This case demonstrates the potential benefits of a comprehensive conservative treatment approach combining chiropractic adjustments, nonsurgical spinal decompression, and therapeutic home exercise for a patient with lumbar disc degeneration, facet arthropathy, and significant mechanical restriction. Following eight weeks of care, R.M. achieved complete pain relief, restoration of normal lumbar function, and measurable improvement in spinal mobility without surgical intervention.
